When you visit our offices, we record personal information such as your name, contact information and date of birth, health history, etc. The personal information we collect will only be used for the purposes originally stated, unless additional consent is obtained.

Dr. Mosher recognizes the importance of being able to reach our patients in a timely fashion, using a variety of communication vehicles, including email, if supplied by our patients. Any personal contact information provided to us will remain confidential.

Updating your information is easy. Each year in January, Dr. Mathew Mosher’s office will send you a notice so we can update our patient records and your contact information.
